The candidate applying for the SEATA Family Scholarship has requested a letter of recommendation from you as part of the application process.  Please complete this form and submit as indicated below.  This information is forwarded directly to the District Nine Scholarship Committee.  Please be as honest and objective as possible. The applicant must request two letters of recommendation.  One should be from a representative of the college/university and the other from a community or church leader.  Both letters should reflect the scholastic achievement, leadership characteristics, and/or social involvement of the dependant.
